Step 1: Complete the Secure Intake Form

Ready to begin care? Start by filling out the encrypted intake form below. It’s short, straightforward, and helps me understand what kind of support you’re looking for—whether that’s ongoing therapy, an autism or ADHD evaluation, or something you’re still figuring out. You don’t need to have all the answers to take this first step.

Step 2: I’ll Follow Up via Secure Encrypted Email

If it looks like we’re a good clinical fit and I have availability, I’ll follow up via secure, HIPAA-compliant email to share next steps. If I’m not currently accepting new clients, I’ll do my best to offer thoughtful referral options—and you’re welcome to request to be added to my waiting list for future openings.

Step 3: Begin the Quest

Once we decide to move forward, I’ll send you onboarding documents to review and sign. These include informed consent forms, a secure intake questionnaire, any release of information you’d like to include, and a Good Faith Estimate outlining the expected cost of services—so you have financial clarity from the start.

If you’d like to use your out-of-network insurance benefits, please contact your insurance provider before your first session to confirm your coverage and ask what documentation they require. I’m happy to provide superbills, but it’s important to let me know ahead of time—I’m not able to create them retroactively.

Your first appointment will be a 60–75 minute intake session, designed to help us begin with intention, clarity, and connection.
